Students enrolled in the Medical Services and Techniques Department, Medical Documentation and Secretarial Program and Medical Imaging Techniques Program in the Fall Semester are required to prepare the documents listed below for hospital practices in their course curriculum by the end of business hours on October 2.

REQUIRED DOCUMENTS

Health Report (Single Physician Report) - Taken within the last 6 months
Blood test result (Eliza tests) - Taken within the last 3 months
Occupational Health and Safety Training Certificate (Must be taken within the last year.)

Note:

All documents must be prepared completely in a single transparent file, and also scanned and made available as a single electronic pdf document. The documents must be submitted within the planning of the instructor teaching the course
